Skilled Nursing Care ~ Provide hands on care by offering a variety of services in the comfort of your private home, assisted living facility, or wherever you call home. We work very close with your physician to obtain the optimum level of health for you. Our goal is for you to live as independent as possible. On our first visit we will explain all services you will be receiving, obtain all vitals, review your medications, conduct a safety assessment to include fall risk, assess your pain, and answer any questions or concerns you may have. We will discuss what your health care goals are. The nurse will then work with our interdisciplinary team of professionals to help those goals be obtained. Because every client is unique, every goal will be customized to suite each individual client.
Home Health Aides ~ Provide hands on care and assistance by helping with bathing, dressing, grooming, and many other activities of daily living.
Therapy Services ~ Physical, Occupational, and Speech Therapists help you restore physical mobility, joint function, assist with daily living skills, and help you to speak and swallow with less effort.
Social Services ~ Will help you with many important items that are so often difficult to accomplish. Some of those items may include helping you to determine if you are eligible for federal, state, or local programs, coordinating Meals on Wheels, where to go for help with medical and utility assistance, medications, estate planning, legal issues, personal counseling services, and so much more.
Dietician Services ~ Meal and diet planning and education.
Wound Care ~ Our nursing and physical therapy services join together to provide optimal wound care.
Catheter Care ~ Managing catheters and ensuring elimination of infections.
Urinary Incontinence ~ Treating pelvic floor dysfunction and incontinence through exercise and modalities as appropriate.
Pain Management ~ With the use of Lymphedema Therapy, Kinesiotaping, Ultrasound, TENS unit, Iontophoresis, Functional Electrical Stimulation, Myofascial Release pain can be greatly reduced and many times completely eliminated.
